This state included the city of Siena and much of southern Tuscany. It grew to become a major economic power during the Middle Ages. Siena was a vital center for finance, commerce, and the arts in Europe. It managed a large territory that eventually included a coastline on the Tyrrhenian Sea. This maritime access helped its commercial policy expand into areas like the Maremma.

The government of Siena changed many times over the centuries. Originally, the city was ruled by a bishop. However, the nobility gained more power during the 12th century to help with territorial disputes. In 1167, the commune of Siena declared independence from episcopal control. By 1179, the state had established a written constitution. In 1286, a new group called "the Nine" began to rule. This council was supported by the Noveschi, a political party of wealthy merchant families.

This era of governance is often called "good governance" by historians.

Siena’s military strength was tested during the Battle of Montaperti. On 4 September 1260, the Sienese Ghibellines fought the Florentine Guelphs.

The Sienese army had about 20,000 men. They faced a much larger Florentine force of approximately 33,000 men. Legend says a thick white cloud covered the battlefield to help the Sienese. In reality, traitors within the Florentine army caused chaos during the fight. This victory helped lead Siena into a "golden age" lasting until 1355.

During this golden age, Siena became a world leader in culture and learning. The city commissioned magnificent buildings like the Siena Cathedral and the Palazzo Pubblico.

The Palazzo Pubblico served as the seat of government. In 1240, the city began supporting the University of Siena through taxes on student housing. By the early 14th century, the university had teachers of law, Latin, logic, and medicine. The city also built massive stone walls to protect its citizens.

These walls were completed in stages, with a second set finished by the late 13th century.

Siena was also a massive center for international finance. The city prospered through the wool trade and money lending. In 1472, Sienese magistrates founded a "mount of piety" called the Banca Monte dei Paschi di Siena. This institution is famous for being the oldest surviving bank in the world. The city’s wealth allowed it to expand its borders into the province of Grosseto. It even secured ports like Talamone and Porto Ercole to manage maritime trade. This economic strength made Siena a rival to other major Italian states.

Political instability and disease eventually weakened the Republic. The Black Death arrived in 1348 and devastated the population. This caused the economy to decline and led to many years of political shifts. Various groups, such as "the Fifteen" and "the Ten," took turns ruling the city. Later, the Petrucci family gained significant influence. Pandolfo Petrucci seized complete control in 1500. Although he was an autocrat, he helped the city recover through his support of arts and sciences. However, internal strife returned after his family's era ended.

The final years of the Republic were marked by intense conflict. The Italian War of 1551–1559 created great pressure on the state. Siena faced a massive siege led by Gian Giacomo Medici. The Sienese people resisted the attack for 18 months. Despite their courage, the city finally surrendered on 21 April 1555. The Spanish King Philip II then gave the territory to the Duchy of Florence. A small group of Sienese families continued to resist in Montalcino until 1559. By the mid-16th century, the Republic's territory covered about 1,100 square kilometers and held 80,000 people.
