The history of this land is very long and complex. Long ago, the Sultanate of Tidore and the Sultanate of Ternate claimed this area. In 1660, the Dutch recognized the Sultan of Tidore's rule over New Guinea. Later, the Dutch and the United Kingdom divided the island in 1824. The western half became part of the Dutch East Indies. The Dutch officially bought West New Guinea in 1901.

Things changed greatly after World War II ended. In 1949, the Netherlands agreed to give control of most former Dutch lands to Indonesia. However, they kept Western New Guinea for themselves. The Dutch argued that Papuans were ethnically different from Indonesians. They wanted to help the Papuan people decide their own future. In 1962, the Dutch agreed to let the United Nations manage the area temporarily. This was part of an agreement called the New York Agreement.

In 1969, a vote was held to decide the future of the land. This vote was called the Act of Free Choice. Indonesia organized this vote to see if the people wanted to join Indonesia. The result said that the territory would become part of Indonesia. However, many people dispute this result. They say only 1,025 hand-picked people were involved in the vote. They also say these people were forced to vote at gunpoint. This was much less than 1% of the people who could have voted.

Since then, there has been much reports of harm in the region. Human rights groups say that Indonesian forces have committed many abuses. Some experts, like scholar De R. G. Crocombe, estimate that 100,000 to 300,000 Papuans have died since the occupation began. A 2004 report from Yale Law School argued these actions fit the definition of genocide. Many people have been killed or forced to leave their homes. In 2022, the United Nations called for urgent aid due to these shocking abuses.

Violence has also heavily impacted Papuan women. A 2013 and 2017 study found that 4 in 10 Papuan women reported state abuse. A follow-up study in 2019 showed similar results. Human Rights Watch describes the region as a de facto police state. They note that peaceful political expression often leads to imprisonment. Indonesia often blocks foreign access to the region. The government cites safety and security concerns for these restrictions. Critics argue this prevents international scrutiny of the situation.

The conflict has broader regional consequences as well. About 13,500 Papuan refugees live in exile in Papua New Guinea. Fighting sometimes spills over the border between the two nations. The Papua New Guinea Defence Force has set up patrols to manage this. Since the late 1970s, the OPM has made threats against business projects in Papua New Guinea. This is often a response to border operations. Many international actors now call for stronger intervention or peacekeeping forces.
