The first written record of the city dates to 1431. At that time, it was a military outpost known as Płoskirów. This name likely came from the Ploska River. Under the Kingdom of Poland, it was a royal city. In 1578, King Stephen Bathory established weekly markets and annual fairs to grow the economy.

Over the centuries, different empires controlled the city. The Cossacks seized the city during the Khmelnytsky Uprising. Later, the Ottoman Empire ruled the area until 1699. In 1793, following the Second Partition of Poland, the city was annexed by the Russian Empire. It was renamed Proskurov during this time. In 1954, the city was renamed Khmelnytskyi. This change honored Bohdan Khmelnytsky, a famous Cossack hetman.

The 20th century brought significant and often tragic changes to the population. During the interwar period, the city was contested by various groups. In 1919, a violent event known as the Proskurov pogrom occurred. This resulted in the deaths of an estimated 1,500 Jews in the city. During World War II, the German Army occupied the city from 1941 to 1944. The Holocaust severely impacted the local community. In 1939, the Jewish population was 42 percent, but by 1959, it had fallen to 10 percent. More than 9,500 Jews were killed in the town during the occupation.

In modern times, Khmelnytskyi remains a vital hub for transportation and education. It is a major rail junction, making it a central point for travel.
