Traditionally, they were grouped in a clade called Tethytheria. This group includes Sirenia, which are manatees and dugongs, and Proboscidea, which are elephants. However, their presence in the Pacific makes their connection to the African-based Afrotheria problematic. Some recent studies suggest they might instead be basal perissodactyls. Perissodactyls are an order of mammals that includes odd-toed animals like horses. The similarities to manatees and elephants might simply be a result of convergent evolution.

Desmostylians eventually disappeared from the fossil record during the late Miocene.

Scientists believe they began to decline during a period of global cooling. These cooling events may have been too severe for them to survive despite their adaptations. Another possibility is ecological competition. As desmostylians declined, the diversity of dugongid sirenians seemed to increase. It is possible that these manatee relatives outcompeted them for the same kelp and seagrass resources. Their disappearance marks the end of a truly unique way of life in the ancient North Pacific.
