Black spent many years teaching and researching new ideas. He taught at the Dental Department at the University of Iowa starting in 1890. Later, he moved to Chicago to continue his work. He was the second Dean of the Northwestern University Dental School. In 1895, he published a special formula for mixing amalgam. Amalgam is a metal used for dental fillings. This formula became the gold standard for almost 70 years.

He created a famous way to group tooth decay. This is known as Black's Classification of Caries Lesions. Dentists still use his system today. To organize the way dentists understood tooth decay, Black created a specific system. This is known as Black's Classification of Caries Lesions. This system categorizes decay based on where it appears on the tooth. His original classification included five distinct categories: Class I involves decay in the pits and fissures of molars, premolars, and the lingual part of anterior teeth. Class II affects the proximal surfaces, or the sides, of molars and premolars. Class III involves the proximal surfaces of incisors and cuspids without involving the incisal angles. Class IV affects the proximal surfaces and includes the incisal angles of anterior teeth. Class V affects the gingival third, or the area near the gum line, of the tooth surfaces. In 1956, W. J. Simon added a sixth category, Class VI, which covers the cusp tips of certain teeth.

Black was also a proponent of preventive measures through his concept of "extension for prevention." This idea suggested that dentists should create extra grooves and pits in a tooth during preparation. The goal was to remove areas where decay might develop in the future. While modern dentistry has shifted toward a focus on minimal intervention, his phrase remains famous. He also conducted important research into the causes of dental fluorosis, which is a condition affecting tooth enamel.

Black's influence extended to the next generation through his son, Arthur. Arthur continued his father's dental research and advocated for the importance of science in dental education. He helped make university education mandatory for dentists. Greene Vardiman Black died of pernicious anemia on August 31, 1915, at the age of 79. His legacy is honored through a statue in Chicago's Lincoln Park. He was also inducted into the International Hall of Fame of Dentistry in 1995.
