Players can choose different ways to play depending on the version. You can play a Single mode alone or a Versus mode with another person. There is even a Double mode where one player uses all eight panels on the stage. Some older games had a Battle mode where players sent distractions to each other. The game also uses a Groove Radar to show how a song is difficult. This radar looks at things like the density of steps and how many jumps you must make.

On the screen, colored arrows scroll upward from the bottom. They move toward a set of stationary arrows near the top called the Step Zone, or receptors. The player must step on the corresponding panels when the scrolling arrows overlap the Step Zone. The game judges accuracy using a scale of results. These range from the highest, Marvelous, down to Perfect, Great, Good, Almost, and finally Miss.

Success in the game is tied to a Dance Gauge, which acts as a life bar. Hitting arrows in time with the music fills this gauge. Failing to hit them drains the gauge. If the gauge is completely exhausted, the player fails the song and the game ends. If the player survives, they reach a Results Screen. This screen shows a letter grade and a numerical score. Depending on the machine settings, a player can usually play up to three songs in a session.

Later versions introduced complex arrow types to increase difficulty. Freeze Arrows are long green arrows that require the player to hold a panel down. The player must stay on the panel until the arrow passes through the Step Zone. Doing this successfully earns an "O.K.!", but releasing too early results in an "N.G." score.

The X series also added Shock Arrows. These appear as walls of arrows with lightning effects. Players must avoid these, as stepping on them results in an "N.G." score. Hitting a Shock Arrow can also hide future steps for a short period.

Players can choose between different modes of play. Single mode is for one person, while Versus mode allows two players to play simultaneously. Double mode allows one player to use all eight panels on the stage. Older versions included Course mode for playing sets of songs back-to-back. There was also Battle mode, where two players used a tug-of-war life bar to send distractions to each other. Some versions even featured Couple or Unison modes for cooperative play.

Difficulty levels have evolved significantly over the decades. Early versions used names like Basic, Another, or Maniac. The SuperNOVA series standardized these to Beginner, Basic, Difficult, Expert, and Challenge. The X series overhauled the numbering system entirely. It moved from a 1–10 scale to a 1–20 scale.

When this change happened, old ratings were roughly multiplied by 1.4 to fit the new scale. For example, a level 9 rating became roughly a 12 or 13. Even with this high scale, no song has yet reached a level 20.

To help players understand song difficulty, Konami introduced the Groove Radar. This tool provides a graphical representation of a song's difficulty across five specific areas. The first is Stream, which measures the overall density of steps. Voltage measures peak density, often seen in songs with a high BPM of 300 or more. Chaos measures "off-beat" steps that do not occur on standard beats. Air measures the number of jumps and Shock Arrows. Finally, Freeze measures the amount of Freeze Arrows in the track.

Finally, the series includes an Extra Stage system to reward high-performing players. If a player clears three songs with a high enough grade, they may earn a free extra song. These songs are often very difficult and may have forced modifiers, such as 1.5x speed. In some cases, players can even reach a "One More Extra Stage." This is a special, highly difficult stage where any missed step or broken combo causes an instant failure. The most recent arcade version, Dance Dance Revolution World, was released on June 12, 2024.
