In 1938, the area expanded through the annexation of the Trans-Olza region. During the German occupation in 1939, administrative divisions changed as Nazi Germany annexed much of the territory. Following World War II, the communist government ended the region's autonomous status. It created the Silesian-Dabrowa voivodeship to reflect both the Silesian and Dąbrowa Basin areas. A major administrative reform in 1999 reduced Poland's voivodeships from 49 to 16, creating the modern Silesian Voivodeship. 
Demographic trends show a shrinking population in recent years. At the end of 2023, the population was 4.32 million, which was a 6% decrease from ten years prior. This decline is driven by an intensifying natural population decrease. In 2023, the total fertility rate was 1.10, which is well below the 2.1 rate needed to sustain a population. 
Religion plays a significant role in the cultural life of the province. It is the most religiously diverse region in Poland. Roman Catholicism is the largest denomination, with 3.063 million adherents. The Jasna Góra sanctuary in Częstochowa is the most-visited shrine in the country. 
Tourism and industrial heritage provide many opportunities for visitors. The Beskid Mountains offer extensive winter sports, including 200 kilometers of ski routes.
