The mechanism of the disease begins with subtle biochemical changes. In healthy cartilage, water content is finely balanced. Compressive forces drive water out of the cartilage. Meanwhile, osmotic pressure draws water back in. This balance relies on collagen fibers and proteoglycans. Proteoglycans create the osmotic pressure needed to hold water. During the onset of osteoarthritis, the collagen matrix becomes disorganized. There is also a decrease in proteoglycan content. This causes a net increase in water content within the cartilage. The loss of collagen outweighs the loss of proteoglycans. Eventually, the cartilage wears away and the underlying bone is affected.

As the disease progresses, several distinct structural changes occur. The ligaments within the joint may become thickened and fibrotic. The menisci, which are pads in the joint, can become damaged or wear away. In some cases, the menisci may be completely absent. New bone outgrowths called osteophytes, or bone spurs, often form on the margins of the joints. These spurs may attempt to improve the surface fit of the joint. The subchondral bone volume also increases. However, this bone becomes less mineralized.

Symptoms of osteoarthritis often progress slowly over many years. The most common symptoms are joint pain and stiffness. Stiffness is most common in the morning.
