These roles include transporting cargo, acting as an airborne command post, and performing reconnaissance. It is also used as an armed gunship in certain configurations. The Mi-8 is officially the most-produced helicopter in history. Over 17,000 units have been manufactured for use by more than 50 different countries. When combined with the related Mil Mi-17, these helicopters represent the third most common operational military aircraft in the world.

The development of the Mi-8 was a complex process of engineering and political negotiation. In 1959, designer Mikhail Mil proposed an all-new two-engined turbine helicopter. This idea followed the success of the Mil Mi-4 and the use of turbines in the Mil Mi-6. Initially, the Soviet military was hesitant to build a new model. They were satisfied with the existing Mil Mi-4. To overcome this, Mil argued that the project was an update to existing turbine technology rather than a completely new design. This strategy persuaded the council of ministers to proceed. In May 1960, an official order was given to create the twin-engine version. This required the creation of purpose-built turbine engines and a new main rotor gearbox designed in-house.

The aircraft evolved through several distinct prototype stages. The first prototype, named the V-8, was based on the Mil Mi-4 but featured a larger cabin. It was a single-engine design that first flew in June 1961. The third prototype, the V-8A, was a major milestone. It flew on August 2, 1962, piloted by Nikolai Ilyushin. This was the first Soviet helicopter to utilize purpose-built gas turbine engines. These were the Isotov TV2 engines, each producing 1,120 kW (1,500 shp). A fourth prototype was designed specifically as a VIP transport. This version used a five-blade rotor instead of four to reduce vibration. It also featured blister perspex cockpit doors and a sliding cabin door. The fifth prototype served as the final model for mass production in the passenger market.

There are several different variants of the Mi-8 family, each with specific features. The Mi-17 is a common export version of the aircraft. While they look similar, there are three main ways to tell them apart. First, the Mi-8 has its tail rotor on the right side, while the Mi-17 has its on the left. Second, the Mi-8 has circular exhausts, but the Mi-17 has oval exhausts. Third, the Mi-17 includes dust shields in front of the engine air intakes and improved armor plating for the crew.

Engineers have also improved the mechanical design of the helicopter over time. Second-generation models moved to a tractor-tail rotor configuration. This change was made to increase yaw authority, which is the ability of the helicopter to turn left or right. In the older "pusher" configuration, the advancing rotor blade moved downwards, which decreased the airspeed across the blade. By switching to the tractor configuration, the upward advancing blades move into the downwash. This increase in airspeed makes the tail rotor more effective at controlling the aircraft's direction.

The Mi-8 has a long and varied operational history across the globe. In the 1970s, the Finnish Defence Forces and Border Guard began using the aircraft. One Mi-8 was lost in Finland in 1982 after it sank through ice during a landing. In Georgia, the helicopters were used by both sides during the War in Abkhazia from 1992 to 1993. The aircraft has also been used by the United Nations in South Sudan. During the Soviet-Afghan War in the 1980s, the Mi-8 was a primary aircraft used by the Soviet Union. Its rugged construction made it easier to maintain and operate in difficult field conditions.

The helicopter has also been used during major industrial and humanitarian crises. In 1986, Mi-8 helicopters were used following the Chernobyl nuclear disaster. They were used to drop over 5,000 metric tons of sand, lead, clay, and boron into the No. 4 reactor to extinguish the fire. This mission was very dangerous, and many helicopters were severely irradiated. Some were eventually abandoned in a "machines cemetery" near the site.
