It serves as the administrative, economic, and royal capital of the state of Terengganu. The city sits at the estuary of the Terengganu River, where the water meets the South China Sea. In the Malay language, the term "kuala" refers to a river mouth, estuary, or confluence. This name accurately describes the geography of the river's broad expanse as it empties into the sea. Notably, it is the only royal capital among Malaysia's nine royal states to share its name with its state.

The city functions through a specific administrative structure. It is the seat of the Kuala Terengganu District, which is the smallest district in the state by area. However, the city area includes the Kuala Nerus District, which was formerly part of Kuala Terengganu. Together, these two districts form the most populous area in Terengganu. In 2010, the combined population was 406,317 people. By 2020, this number had declined to 375,424.

History shows that the region has been a significant maritime location for centuries. Early Chinese historical sources from the Sui dynasty mention a state called Tan-Tan that sent tributes to China.

This state likely occupied the Terengganu region. Following the decline of the Sui dynasty, Tan-Tan continued to send tributes to the Tang dynasty. Later, the area fell under the dominance of the Srivijaya empire during the 7th century. Chinese texts from the 12th and 13th centuries, such as the Lingwai Daida, refer to the area as Teng-ya-nu. After the fall of Srivijaya, the region came under the influence of the Majapahit empire.

The modern Sultanate of Terengganu was formally established in 1708. The first Sultan, Zainal Abidin I, established his court near Kuala Berang. He moved his court several times before eventually settling near Bukit Keledang. During the early 18th century, the town was quite small, containing roughly one thousand houses.

At that time, the Chinese population made up about half of the residents. These inhabitants were primarily involved in agriculture and trading activities. This long history as a port helped mix Malay influences with other cultures.

Political shifts shaped the city during the 19th and 20th centuries. In 1831, a civil war broke out between two claimants to the throne, Tengku Mansur and Tengku Omar. After various conflicts, Tengku Omar eventually reclaimed the throne as Sultan Omar. In 1862, the British attempted to intervene regarding the presence of the ex-Sultan of Riau-Lingga. This led to a military response where British ships bombarded Kuala Terengganu.
