This linguistic skill was evident in her 1589 play, La Pazzia d'Isabella. In this work, she used different languages and imitated various dialects to create a sense of madness. She was also a successful playwright who addressed complex social themes. In her 1588 play, Mirtilla, she explored the independence of women and deep female relationships. She even used the character Ardelia to explore the idea of lesbianism.

Andreini's writing was respected by the highest levels of society.

In 1601, she was admitted to the Accademia degli Intenti, a literary society in Pavia. She earned the nickname "Accesa" within this group. She also competed in a prestigious poetry contest held by Cardinal Giorgio Cinthio Aldobrandini. In that contest, she achieved second place, trailing only the famous poet Torquato Tasso. Her literary output was vast, including a collection of 359 poems titled Rime, published in 1601. Her works often discussed gender identity, self-expression, and the body.

Her influence extended into the world of music and poetry long after her career. Many composers and poets wrote tributes to honor her talent. The poet Gherardo Borgogni exchanged verses with her for nearly twenty years. These verses later became lyrics for musical tributes by composers like Leone Leoni. Other musicians, such as the English composer Peter Philips, also created works in her honour. Her reputation was so great that she was often addressed as the "star" of her company. Even Marie de' Medici, the Queen of France, referred to her in letters.

Isabella Andreini died in early 1604 near Lyon while traveling back to Italy.

Her death was marked by a state funeral. To commemorate her life, special memorial coins were produced. One side of these coins depicted her as a powerful Roman ruler. The other side featured the Roman goddess Fame and the words "aeterna fama," meaning eternal fame. Her legacy lives on through the study of her posthumous writings and the continued use of the Isabella character in theatre. She proved that women could achieve international fame through both performance and intellect.
