The earliest written records come from the Yuan Dynasty in the 1300s. These records mention Oton as a trade outpost. Spanish settlers arrived in the area in 1566. They established it as the second Spanish colonial center in the Philippines. This followed the establishment of Cebu. The city was founded by 80 Spaniards from Europe. Over time, more Spanish and Mexican soldiers arrived to reinforce the area. By the 1700s, the area was home to thousands of native families.

The city earned a very special honor from Spain. Queen Regent Maria Christina of Austria gave it a title. She called it "La Muy Leal y Noble Ciudad." This means "Most Loyal and Noble City." She gave this title because the city stayed loyal to the Spanish crown. This happened during the Philippine Revolution. Iloilo City even served as the last capital of the Spanish Empire in Asia. This was before the Treaty of Paris in 1898. That treaty ceded the Philippines to the United States. At the start of the 20th century, it was the "Queen City of the South."

Local culture is shaped by both history and legend.

The Maragtas epic tells a story of ten datus from Borneo. Led by Datu Puti, they supposedly fled a tyrant to reach Panay. They are said to have bought the lowlands with gold. While historians view the Maragtas as a 20th-century invention, it remains a strong local tradition. This legend inspires the Dinagyang Festival held every January. This festival honors the Santo Niño.
