The toxin works by disrupting the way nerves communicate with muscles. This process is known as a neuromuscular blockade. To move a muscle, your body must release a chemical called acetylcholine. This chemical travels from the nerve to the muscle to trigger movement. The botulinum toxin blocks the release of acetylcholine from the presynaptic membrane. Without this chemical signal, the muscles cannot receive instructions to contract. This results in paralysis, which is the loss of muscle function. This is followed by descending paralysis. The weakness moves from the head down to the shoulders and arms. Eventually, it reaches the thighs and the feet.

Infant botulism is the most common form in the United States. It happens when infants under one year old ingest the spores. The bacteria then grow inside the child's small intestine. This can happen if a baby consumes honey, which is a known reservoir for these spores.

Medical care has changed significantly for patients over the last 50 years. In the past, about 50% of people with botulism died. Due to better supportive care, the death rate has fallen to about 7%. Doctors treat the illness with an antitoxin to fight the toxin. If the toxin paralyzes the muscles used for breathing, a patient may need mechanical ventilation. This machine helps the person breathe for themselves, sometimes for many months. Patients may also require intensive nursing care and rehabilitation therapy to recover.

Prevention relies heavily on understanding how to destroy the bacteria and the toxin. Heating food for more than five minutes can destroy the toxin itself. However, the tough spores require much higher temperatures to die. For example, an autoclave uses moist heat at 120°C for 15 minutes to kill spores. Dry heat at 160°C for two hours is also effective. Commercial canning uses specific heat levels to inactivate Group I strains. Group II strains are slightly less resistant and may be damaged at 90°C for 10 minutes. Following strict food safety and preparation rules is the best way to stay safe.
