Geologists use several different theories to explain how this plateau formed. One hypothesis suggests that weaknesses existed in the Earth's crust before tectonic shortening occurred. These weaknesses might have split tectonic deformation and uplift into the eastern and western cordillera. This would leave space for the Altiplano basin to form. Another theory involves magmatic processes rooted in the asthenosphere. These processes may have contributed to the uplift of the plateau. Some scientists also suggest that the convective removal of dense lower lithosphere caused the region to float higher through isostasy.

The Altiplano is characterized by endorheism, which means it has an inland drainage system. In this system, water does not flow out to the oceans. Instead, it collects in internal basins. Climate has played a major role in this process. Climate helped determine the formation of internal drainage and sediment trapping within the Andes. This may have blocked tectonic deformation in the central area between the two mountain ranges. This process can expel deformation toward the flanks of the mountain system.

During the Pleistocene epoch, the landscape looked very different. Both the northern and southern parts of the Altiplano were covered by vast pluvial lakes. Today, only remnants of these ancient lakes remain. Lake Titicaca sits on the border of Peru and Bolivia. It is the largest lake in South America. Another former lake was Lake Poopó in Bolivia. However, Lake Poopó dried up completely by December 2015 and was declared defunct. The large salt flats known as salares, such as Salar de Uyuni and Salar de Coipasa, formed after these paleolakes dried out.

The geography of the plateau is defined by extreme elevations and varied climates. The average elevation is about 3,750 meters, or 12,300 feet. Because of this height, the air is hypoxic, meaning it has low oxygen levels. To the west, the Central Volcanic Zone contains massive active volcanoes. These include Parinacota, which is 6,348 meters high, and Uturunku, which is 6,008 meters high. In the northeast, the Cordillera Real features peaks like Illimani at 6,438 meters.
